You can safely take the salinity up to .6% by using the same method as usual (increments every 12 hours), but I would not recommend keeping it there for a very long time, only just enough time to cure the disease you are treating for. I also would not use this level with very tiny or weak fish.
It is not necessary to use .6% to treat ordinary salt-susceptible organisms such as ich, but some organisms have developed salt resistance - there is a salt-resistant strain of costia around for example - and in these cases it is necessary to go up to .6% or use a specific medication.
Many German fishkeeping books used to routinely suggest .9%.
